The Lodges at Oak Grove is more than a care facility—it’s a place where family meets hospitality and healthcare meets heart. Our story begins in 1976 with a humble nursing home serving the Oak Grove community. By 1985, our rehab services had begun in a dedicated wing, and in 2013, we fully renovated to elevate care and comfort alike. But it was in 2020 that we envisioned something radically new: a boutique rehabilitation experience set apart from conventional nursing homes.
Amidst the challenges of hurricanes and the COVID-19 pandemic, we broke ground on a state-of-the-art, lodge-style facility. The result? A warm, welcoming environment designed to feel more like a hill country retreat than a traditional care center. Nestled beneath century-old oaks and filled with natural light, The Lodges offers modern amenities, exceptional therapy spaces, and private accommodations—all operated independently and without corporate red tape.
Meet Our Team
At The Lodges at Oak Grove, we believe strong leadership fosters trust, stability, and innovation. Our team includes experienced administrators, directors of nursing, and department heads who have dedicated their careers to compassionate elder care. Many of our staff members have been with us for a decade or more, forming lasting relationships with residents and families. Our leadership believes deeply in service, dignity, and the healing power of environment and empathy. We have always led with quality care, and now, we proudly match that excellence with an equally remarkable space.

Blaire Lewis
Blaire is the Speech-Language Pathologist at Oak Grove Nursing Home. She received her Bachelor of Science degree in Speech and Hearing Sciences in 2018 and her Master of Science in Speech-Language Pathology in 2021 from Lamar University.
Over the past few years, Blaire has focused her clinical skills on evaluating, diagnosing, and treating adult and geriatric patients with disorders in motor speech, language, cognitive-linguistics, swallowing and voice. She is certified in Ampcare ESP, an effective swallowing protocol that combines neuromuscular electrical stimulation with resistive exercises and restorative posture device to improve the swallow physiology in those with dysphagia.
She holds her Certificate of Clinical Competence with the American Speech, Language, and Hearing Association (ASHA), is a member of the Texas Speech, Language, and Hearing Association (TSHA).
Daniel Duplechin
Daniel chose the healthcare field because of his great passion for helping others. With over 15 years of experience in both hospice care and nursing facilities, Daniel lives by his favorite quote of Confucius – “Choose a job you love and you will never have to work a day in your life”.
Daniel’s personal mission is to create, lead and inspire a compassionate team of caregivers and professionals who love and respect all Oak Grove residents.

Maria Padilla
Maria earned her Bachelor of Science in Physical Therapy in the year 2000 in the Philippines. She spent her early years volunteering and working at a tertiary hospital that provided both inpatient and outpatient care. In addition, she was a freelance home health therapist while working for a non-profit organization that catered to the rehabilitation needs of children and adolescents with disabilities.
After immigrating to the US in 2011, she began working in a skilled nursing facility that provided both inpatient and outpatient rehab services. She decided to continue her studies, so she enrolled in a postgraduate program. In 2020, she earned a Doctorate in Physical Therapy specializing in Manual Therapy.
In August 2022, she began working for Premier Rehab LLC. She was nominated for Employee of the Year and Clinical Excellence of the Year 2022. Maria’s hard work and dedication earned her the Clinical Excellence of the Year Award in 2023. With over 22 years of experience in the field of physical therapy, Dr. Maria Alicia Padilla’s approach is rooted in evidence-based practices, ensuring each patient receives the best possible care.
